1. GTOPI patent pending OandP CustomRestoration Services are now being purchased by the Veterans Affairs VA PSAS. This is covered service at 100% with a qualified ‘Consult Prescription’ when written by your VA Physician at the Amputee Care Clinic or a VA Psychiatrist can also write your ‘Consult Prescription’.
Notify the VA representative you wish to have GTOPI provide for your custom cosmetic restorations.
Note: Be sure to have GTOPI Duns # 024489295 available when you talk with your VA Hospital respresentative.
2. Ask your VA Physician to fax your ‘Consult Prescription’ to GTOPI Fax # (360) 876-7733
3. After we receive the faxed ‘Consult Prescription’, GTOPI can begin processing your custom cosmetic restoration request with your local VA Hospital. Note: If you receive care from a VA Prosthetist Contractor, provide the clinic name, address, fax and CPO contact information and we will forward your custom request to his/her office.
